Fees
Therapy and Consultation: $160 for 45-50 minute session $180 for 60 minutes.
Assessment: $200 per hour
Trainings: Determined by the length and preparation time

Travel
If Dr. Milhem needs to travel in order to provide services, the client will be charged for the time spent traveling to and from the location. Travel time is not reimbursable by insurance companies.
Consultation, Phone Calls, Emails, and Collateral Contacts
In many cases collaboration with schools and other providers is very important in order to provide comprehensive intervention services. In the case that Dr. Milhem or another provider need to consult, the client is responsible for payment. The client must authorize this exchange before it occurs. In many cases, clients require extra attention, whether by email or on the phone. Clients will be billed for conversations that exceed 15 minutes. Phone calls, consultation and collateral contacts are not reimbursable services by insurance companies.
Sessions
Therapy sessions are typically 45-50 minutes in length, which gives Dr. Milhem 10 minutes to complete paperwork and administrative tasks for that client. If the client feels that they need a longer session, they are encouraged to plan with Dr. Milhem to have a 60 or 90 minutes session scheduled.
Cancelation
Dr. Milhem requires 48 hours notice, in writing(via email) if you wish to cancel an appointment. This allows Dr. Milhem to see individuals on the waiting list or offer the spot to other clients. If the client does not cancel within 48 hours notice, the client will be billed for that appointment. INSURANCE DOES NOT COVER MISSED APPOINTMENTS. Special cases may arise, and will be assessed accordingly.
